RiverSpring Health Plans serves over 18,000 frail and disabled individuals, empowering them to live independently in their homes for as long as possible.

 

As the Social Worker, you will provide direct services to those MLTC clients/families that require assistance with social, economic, long term planning, counseling and/or motivation fo increased independence.

Responsibilities

Key Responsibilities:

  • Performing initial psychosocial assessments and
  • Follow-up evaluations of members to determine needs, coping skills, barriers to care and social determinants of health
  • Responsible for gathering pertinent social information from member's families or agency staff and maneuver conferences, etc. 

Qualifications

Qualifications:

  • NYS License to practice Social Work
  • Master's degree in Social Work from a school accredited by the Council on Social Work Education and/or the NYS Department of Education
  • One year of experience in the provision of social work in an acute care or health care facility/organization
  • Experience in a home care organization preferred
  • Ability to read, analyze, and interpret general business correspondence,a nd procedure manuals
  • Ability to effectively present information and respond to questions from managers, clients, customers, and the general public. 
  • Willing to travel to all Borough's if needed
  • Bilingual Russian preferred

Compensation:

  • Annual Salary: $80,000-$85,000
