RiverSpring Certified Home Health Agency provides the acutely ill patient with short term skilled home visits: nursing, physical therapy, occupational therapy, speech therapy, and social work. 

The CHHA - Fee For Service home care RN conducts home visits providing skilled clinical assessments, patient education, and direct nursing care to their patients in their own homes. The home care RN coordinates skilled services within the patient’s interdisciplinary team as well as directs the patient’s paraprofessional services.

The ideal RN candidate is a dedicated, compassionate, and confident clinician with a current NYS RN license and strong organizational, communication, written, and customer service skills. We expect our nursing candidate to be competent with Microsoft office, and have a reliable mode of transportation in order to conduct home visits. A BSN degree, prior CHHA experience, and a working knowledge of Home Care Home Base EMR are preferable.

Start of Care:    $175.00 - 7 Days A Week *

Revisit ALL:        $ 80.00 - 7 Days A Week

Resumptions:    $ 100.00 - 7 Days A Week

Recertification: $ 100.00 - 7 Days A Week
