The CHHA-Clinical Manager will collaborate with the interdisciplinary team’s Plan of Care to ensure necessary, appropriate and effective care for the patient supervising Nurse Care Managers. Develops and implements orientation programs for field staff and ongoing in-service education. Participates in compliance and quality improvement efforts. Oversees the day to day operations in the absence of the Director of Patient Services. Provides oversight and supervision to the Nurse Care Managers, Nutritionist, Patient Services Liaison, Paraprofessional Scheduling Supervisor and Admissions Liaison as well as other patients of the clinical care team as deemed appropriate in provision of care.
Current New York State Registered Nurse License and BSN degree or New York State Registered Nurse License and degree in Health & Human Services with 2 years of Home Care experience required. Master’s degree preferred. Five (5) years assessing patients for home care safety and developing appropriate care plans and plan adjustments experience required. Two (2) years supervisory experience required. Patient Review Instrument (PRI) assessor skills experience preferred. Valid driver’s license required. MS Office required. Must be able to read, write, and speak English. Ability to read, analyze, and interpret general business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Compensation: $130,000 min. to $140,000 max. Annually
